How to use the IRMAA Calculator 2026
- Pick the filing status from your 2024 tax return.
- Enter your 2024 modified adjusted gross income, which is adjusted gross income plus tax-exempt interest.
- Read your 2026 Part B premium, the Part D surcharge added to your drug plan premium, and how far you are from the next tier.
How the premium is set
IRMAA is a set of tiers, not a percentage. Your modified adjusted gross income from two years earlier decides which tier you are in, and everyone in a tier pays the same amounts. The page finds your tier in the 2026 table published by the Centers for Medicare and Medicaid Services.
tier = where 2024 MAGI falls in the 2026 table for your filing statusPart B = $202.90 + Part B surcharge for that tierPart D = your plan's premium + Part D surcharge for that tieryearly extra = (Part B surcharge + Part D surcharge) × 12, per person
Married people who file separately and lived with their spouse at any time in the year have only three tiers: the standard premium up to $109,000, and the two highest tiers above it. That makes separate filing expensive for Medicare purposes.
With your numbers
- 2026 premiums use the modified AGI on your 2024 tax return, two years earlier
- Every tier is a cliff: one dollar over the line pays the whole surcharge for that tier

Common questions about the IRMAA Calculator 2026
- What is IRMAA?
- The income-related monthly adjustment amount: a surcharge on Medicare Part B and Part D premiums for people with higher incomes. Social Security sets it each year from the income on your tax return and takes it from your benefit or bills you for it.
- Why does 2026 depend on my 2024 income?
- Social Security uses the most recent tax return the IRS can give it when premiums are set in the autumn, which is the return from two years earlier. So 2026 premiums use 2024 income, and income you take in 2026, such as a large IRA withdrawal or a home sale, will affect your 2028 premiums.
- What are the 2026 IRMAA brackets?
- For single filers the standard $202.90 Part B premium applies up to $109,000. Above that it is $284.10 to $137,000, $405.80 to $171,000, $527.50 to $205,000, $649.20 below $500,000 and $689.90 from $500,000. Joint filers' limits are double, with the top tier from $750,000. Part D surcharges run from $14.50 to $91.00 a month.
- Is it a cliff?
- Yes. One dollar over a limit adds the whole surcharge for that tier, for every month of the year. Someone just over a line may be able to bring income below it with timing of withdrawals, Roth conversions or capital gains, two years before the premiums it affects.
- Can I get IRMAA reduced?
- If your income dropped because of a life-changing event, such as retirement, marriage, divorce, the death of a spouse or the loss of a pension, you can ask Social Security to use a more recent year on Form SSA-44.
